Acceptance Criteria
RISE accepts residents exclusively through approved agency referrals. Every placement is reviewed to ensure the right fit for the resident and the household.
Men 18+
The RISE program is designed specifically for adult men who are ready to engage with structure, meet household expectations, and work toward greater independence. We serve men transitioning from reentry, probation, or parole supervision, behavioral health systems, and other adult service programs. Residents must be able to live independently in a shared setting without on-site clinical supervision or 24-hour staffing support.
Agency Referred
All placements are coordinated exclusively through approved referring agency partners. RISE does not accept walk-in placements, self-referrals, or private-pay arrangements outside of an established agency agreement. Residents are never billed directly — all housing costs are invoiced to the referring agency, keeping the financial relationship clean, professional, and entirely between RISE and your organization.
Active Agency Engagement
Residents are expected to remain actively connected to their referring agency, case manager, probation officer, or supervision team throughout their placement. Compliance with all agency requirements, court conditions, and supervision obligations is a condition of continued housing. RISE documents placement activity, incidents, and conduct concerns and communicates them to the referring agency in a timely and professional manner — keeping your team informed at every stage.

Check Availability
Contact RISE to confirm current bed availability and discuss whether the placement is a good fit before submitting a referral.
Submit a Referral
The referring agency submits a completed Agency Referral Intake Form with all required client information, supervision conditions, and funding authorization.
Eligibility Review
RISE reviews the referral within two business days and communicates the placement decision in writing along with any questions or concerns.

Confirm the Agreement
Move-in date, monthly rate, billing terms, and agency point of contact are confirmed in writing. New agencies complete vendor setup at this stage.
Move-In and Orientation
The resident completes move-in orientation, reviews the Resident Binder, and signs the Occupancy Agreement. The referring agency is notified of successful move-in the same day.
Ongoing Partnership
RISE maintains open communication with the referring agency throughout the placement, including incident notification, discharge planning, and monthly invoicing.

Seamless Billing
RISE is built for agency partnerships, and our billing model reflects that. Residents are never charged directly. All housing costs are invoiced monthly to the referring agency through a straightforward agency-pay model that keeps the financial relationship clean, professional, and entirely between RISE and your organization. Invoices are submitted on the first business day of each month for the prior month's occupancy, itemized by resident and dates of service. Partial months are prorated from the confirmed move-in date. Payment terms are net 30 days from the invoice date. We accept ACH and check, and we are prepared to complete your agency's vendor setup process including W-9 and certificate of insurance documentation.
Transparent Communication
RISE believes that what happens in the home should never be a surprise to the agency that placed a resident there. We maintain proactive, professional communication with our agency partners at every stage of the placement. You can expect referral inquiries acknowledged within one business day, placement decisions communicated within two business days of a complete referral, same-day notification of successful move-in, incident notification within 24 hours of any reportable event, advance written notice of any non-emergency discharge with a minimum of 72 hours, and monthly invoicing on a consistent and predictable schedule.
